Registration *As of 22nd July 2022 we are currently not accepting new patients at the practice. please check this page again in a few months for changes to this.*

New Patients are always welcome here at Yew Tree Dental Care. (At present during Covid-19 pandemic we are operating a waiting list system. There will be a minimum of 1 month wait, but this is not a guarantee and it may be much longer. However, we may contact you at short notice on the day or day before, if we have any cancellations, then you may be seen much sooner. If you require a certain day/time then unfortunately you may be waiting much longer to be seen. If you are happy to be put onto the waiting list then see the instructions below)

We look forward to welcoming you and your family to Yew Tree Dental Care in the very near future. If you would like to register yourself or a child, you need to email us at info@yewtreedentalcare.co.uk and request new patient registration information.

Once you have emailed us, we will send you an information sheet with a list of required documents that you need to email back to us. You will then receive an email with a link to fill out some important NHS forms, that you will need to read carefully, fill out and sign electronically on your mobile phone or computer or tablet.

We can only register you on the system once these forms are completed correctly and you will need to call or email us once you have done these to be put onto the waiting list.

You can register for a free email address with Google, Microsoft or your internet provider, or you can ask a family member for theirs if you cannot register one. 

Please be aware that your registration is not complete until you attend the practice for an appointment and you will not be entitled to any emergency appointments if you have only registered and not attended. You are required to attend the dental practice for your regular check-ups every 6 months or as requested by your dentist to remain active on our system.  If you do not, then you will be deactivated again and may not be able to get an appointment when you need one. 

In cases of repeated deactivations, we may have to remove you from our list permanently and you will have to register with another dental practice.  You will receive reminders by email/SMS and/or letter to attend so please respond as soon as you receive any of these.
